Intake Forms
For your convenience, we have provided the forms to be completed prior to your first session in PDF format for you to download and fill out. Each therapist has his or her own set of paperwork, so please be sure to download the correct forms. If you have any questions while filling out your paperwork, please feel free to contact our office at (479) 442-8900. Bring these completed forms to your first session, along with your insurance card (if applicable). You will still need to arrive at least ten minutes early for your initial session.
